Required Forms

Before your new client appointment, you will need to fill out a series of forms online. It is important that you fill these out to be aware of your privacy rights and that you are fully aware of our practice’s policy and procedures. If these forms are not completed prior to the visit, you cannot be seen.

New Client Forms

As a new client, you will be required to complete a number of new forms. These forms will be sent to you via email or can be found of the client portal once we have scheduled your new client appointment. If these forms are not completed prior to the visit, you cannot be seen.

Telehealth Consent Form

This form authorizes the use of telehealth platforms for initial evaluation and follow up visits. Please complete and bring this form with you.

HIPAA Privacy Policy

This form reviews privacy protection and patient rights with regards to the use and disclosure of protected health information. Please complete and bring this form with you.

Clinician Referral
